3 Effective Fertility Treatment Options to Consider Before You Try IVF

fertility treatments before IVF

When you are struggling to conceive, it is natural to want the quickest, most direct path to a baby. For many, the mind jumps immediately to in vitro fertilization (IVF). However, you should know that, while IVF is a revolutionary and highly successful technology, it is also physically demanding, time-intensive, and a significant financial investment.

The reality is that IVF is not always the first line of defence. In fact, many people achieve their dream of parenthood through less invasive interventions that work with the body’s natural processes.

Before committing to a specific path, the most critical step is a comprehensive fertility assessment. At ONE Fertility Kitchener Waterloo, we do not believe in a one-size-fits-all approach. Your journey begins with identifying the root cause of your fertility challenges. Based on those results, we may recommend the effective fertility treatments before IVF that are best suited to your unique physiology and goals.

Let’s review the three main categories of pre-IVF treatments available at our clinic.

Option #1: Ovulation Induction & Timed Intercourse

For many women, the barrier to conception is simply timing. If you have irregular cycles or a condition like polycystic ovary syndrome (PCOS), your body may not be releasing an egg consistently, making it nearly impossible to predict the fertile window. Ovulation induction (OI) is often the simplest and least invasive starting point.

How It Works

OI typically involves the woman taking oral medications, such as letrozole or clomiphene citrate, for a few days at the beginning of her cycle. These medications gently signal the pituitary gland to produce more hormones, encouraging the ovaries to mature and release one or two healthy eggs.

However, taking medication is only half the equation. At ONE Fertility, we emphasize the necessity of cycle monitoring.

Through transvaginal ultrasounds and blood work, we track the growth of your follicles in real-time. This ensures we are not overstimulating the ovaries (which can lead to a risk of multiples) and allows us to pinpoint the exact moment ovulation is about to occur.

Once the monitoring indicates the egg is ready, we advise the couple on the precise window for “timed intercourse.”

When It’s the Right Fit

This option is primarily designed for women with ovulation disorders, irregular menstrual cycles, or mild PCOS. It is generally not recommended if a woman has blocked fallopian tubes or if the male partner is experiencing significant male factor infertility.

The ONE Fertility Advantage

We move beyond simple calendar guessing. By combining medication with expert monitoring, ONE Fertility removes the uncertainty from the process, maximizing your chances of success while minimizing risks.

Option #2: Intrauterine Insemination

If OI is about timing, intrauterine insemination (IUI) is about shortening the journey for the sperm. When exploring fertility options before IVF, IUI is often considered the “mid-level” intervention—more targeted than timed intercourse but significantly less invasive than IVF.

How It Works

IUI involves placing prepared sperm directly into the uterus, bypassing the cervix to get the sperm closer to the egg. Here’s our three-step process:

  1. Stimulation: Similar to OI, the clinic may use oral medication or low-dose injectable medications to stimulate the growth of follicles.
  2. Sperm Preparation: This is the critical differentiator. On the day of the procedure, the sperm sample is processed in ONE Fertility’s world-class laboratory. Through a technique called “sperm washing,” we remove seminal fluid and isolate the healthiest, most motile (moving) sperm. We then concentrate these top-tier sperm into a small volume.
  3. Insemination: Using a thin, flexible catheter, the specialist places the concentrated sperm directly into the uterus. The procedure is quick—usually taking only a few minutes—and causes minimal discomfort, similar to a Pap test.

When It’s the Right Fit

Because IUI is so versatile, it is often at the top of the list of recommended fertility treatment options before IVF for:

  • Mild male factor infertility (slightly low count or motility)
  • Unexplained infertility (typically attempted 3–6 times)
  • Cervical mucus issues (where the environment of the cervix is hostile to sperm)
  • Single women or same-sex couples utilizing donor sperm

The ONE Fertility Advantage

Success in IUI relies heavily on the quality of the sperm preparation. Our advanced laboratory setting ensures that only the highest quality sperm are introduced to the uterus, significantly improving the odds of fertilizat ion compared to natural intercourse.

Option #3: Surgical Correction

Sometimes, the barrier to conception is structural. Before beginning any cycle of medication, it is vital to ensure the physical environment—the uterus and fallopian tubes—is capable of facilitating a pregnancy. If the clinic’s initial diagnostics reveal a physical blockage or abnormality, minimally invasive surgery may be required.

How It Works

There are two primary surgical approaches used to restore fertility:

  • Hysteroscopy: This procedure involves inserting a thin, lighted tube through the vagina and cervix to examine the inside of the uterus. No incisions are made. A hysteroscopy is used to remove polyps, small fibroids, or scar tissue that might be preventing an embryo from implanting.
  • Laparoscopy: This is a “keyhole” surgery performed through small incisions in the abdomen. It allows the surgeon to view the outside of the uterus, ovaries, and fallopian tubes. Laparoscopies are commonly used to diagnose and treat mild endometriosis or to open blocked fallopian tubes.

When It’s the Right Fit

Surgical correction is necessary when diagnostic tests, such as a saline sonohysterogram (SHG) or hysterosalpingogram (HSG), reveal structural issues that medication cannot fix.

The ONE Fertility Advantage

The ONE Fertility Kitchener Waterloo team includes highly experienced fertility specialists, as well as a physician with advanced surgical expertise when surgery is part of the best path forward. By addressing underlying physical concerns when needed, we can often improve the effectiveness of simpler fertility treatments or help enhance outcomes if advanced reproductive technologies become part of your care journey.

When to Move on to IVF

While we advocate for the least invasive method possible, there is a tipping point when moving on to IVF becomes the most logical and successful medical decision. So, how long should you have been trying to conceive before IVF becomes the necessary next step?

While each individual’s answer is truly personal, there are objective criteria that may lead to us recommending bypassing or moving on from the treatments already listed above and going to IVF as the next step:

  • Severe Male Factor Infertility: If sperm counts are extremely low, IUI may not be effective.
  • Blocked Tubes: If both fallopian tubes are blocked or damaged, the egg and sperm cannot meet naturally.
  • Advanced Maternal Age: If time is a critical factor, moving directly to IVF may offer the highest success rate per cycle.
  • Treatment Fatigue: If a patient has undergone three to six cycles of IUI or OI without success, the statistical likelihood of these methods working decreases.

Ultimately, this decision is a partnership. We’re here to share the medical insight and information you need, but the decision is always yours, supported by specialists who help you weigh every option with care for your time, emotional wellbeing, and financial investment.
